Description

We are currently seeking a conscientious, motivated individual to join our team as a Kentucky Veterans Center Social Worker Specialist. We are committed to serving Kentucky’s Veterans by fostering independence, preserving dignity, and providing compassionate care.


The candidate will assist in coordinating, evaluating and implementing the social service program at Thomson- Hood Veterans Center.


Other responsibilities include but are not limited to the following:

• Provides professional social work and counseling services to the veterans and facility staff.
• Coordinates resident interaction with family and community support groups.
• Monitors the residents’ social and emotional state, adjustment to the nursing home environment and treatment progress.
• Documents psychosocial histories, treatment plans, discharge plans and other pertinent data.
• Other duties as assigned.

Minimum Requirements

EDUCATION: See Special Requirements.

EXPERIENCE, TRAINING, OR SKILLS: Two years of professional social work experience.

Substitute EDUCATION for EXPERIENCE: A master's degree in social work will substitute for one year of the required experience.

Substitute EXPERIENCE for EDUCATION: NONE

SPECIAL REQUIREMENTS (AGE, LICENSURE, REGULATION, ETC.): Must be licensed in Kentucky as a social worker by the Kentucky Board of Social Work. Must maintain any required licensure (s), certification(s), or other credentials for the length of employment in this job classification. Employing agency is responsible for ensuring employee possesses and maintains required licensure(s), certification(s) or other credentials.

Working Conditions

Incumbents working in this job title typically perform duties in an office setting.

Probationary Period

This job has an initial and promotional probationary period of 6 months, except as provided in KRS 18A.111.
